I am writing a thesis right now. I am using EndNote X1, whick is compatible with Word2007. I imported APA 5th (Theses) from EndNote X1 and started writing on it. However, when I tried to build TOC, it only shows incomplete heading. Heading 4 doesn't appear. According to the APA 5th, if I use three level, I have to use Heading 1, 3 and 4. Create your table of contents.
Right click on a TOC element select Edit Field
Under categories select (All)
Under Field Names Select TOC
Click Table of Contents button
Under General section select number of desired levels to be for Show Levels:
Click OK to close window and update the TOC

This thread was very helpful. I just wanted to add this Mac Office 2008 note.
Right click and then click on "Toggle Field Codes."
Then edit the field. Change the 1-3 to 1-4.
I suppose you could edit it to however many heading you wish...
