Rename .img or .iso image to 8-symbols name, add this image to a BT\boot folder on this disk, and end.
This 8-symbols name of image == 8-symbols boot menu item. If you want see a 9-10 or more symbols name in the menu item
add line ex. if image SOMENAME.IMG then SOMENAME = You Like line in BT\TREE2WBA.INI in ASSOCIATION section

[ ASSOCIATION ]
SOMENAME = You Like line

and menu item will become such - You Like line

If you add you programm (programm.exe) to a .uha or .cab arcive( progarc.uha), and add in this archive .BAT file
(progarc.bat), then if select item in the menu (progarc), then archive extracting and file running.
