The answer lies with the firewall. Some firewalls have a text log of all traffic "in and out" with the information you seek. Click the Help section of your firewall or reference the documentation that accompanied the software for details.

you need a good port sniffer.

Be carefull with port sniffers. Some ISPs log the activity. Make sure the sniffer does not activate itself if you connect to the internet. BWTH, Black Ice Defender is a good start for beginners.
